What to Bring to Your Consultation

To help us provide the best possible care and make the most of your appointment, please bring the following:

• A valid referral from your doctor (required to receive a Medicare rebate)

• Your Medicare card and any applicable DVA or Pension Card

• Details of your private health insurance (if applicable)

• Recent blood test results and imaging reports (CT, ultrasound, MRI or X-rays), where available, as well as any previous endoscopy reports and pathology results

• Correspondence from other specialists and details of any doctors involved in your care

• A current list of your medications, including dosages
